  6. Add two references, with their full name, relationship, company, phone, and address
  7. Add Any relevant equipment with which you have experience
  8. Add your relevant skills 
  9. Add up to 3 of your schools of graduation, including college, high school, and other trade schools
  10. Add your military service (if you are not a United States Veteran, select no on the Military Service page) 
  11. Add your criminal history (if you do not have a criminal history select no on the Criminal History page) 
  12. Return to this page, complete the fields electronically, and sign and date below.
